Visa information

All foreign students intending to study in Collegium Medicum are required to obtain a document that would prove their staying in Poland to be legal (registering of residence).

Procedure of registering the residence depends on the student's citizenship:
 

Students from countries that are members of European Union + Norway*

WHAT DO YOU NEED TO APPLY

  1. application form – (must be completed in Polish)

  1. copy of passport – page with a photo (the original is to be shown at the Office)
  2. copy of European Health Insurance card (the original is to be shown at the Office)
  3. confirmation of being a Collegium Medicum student (it may take up to 5 days for the documents to be signed)
  4. confirmation of student’s ability to support himself/herself in Poland (bank statements from the student’s bank account (in Poland), granting the loan confirmation, income statement)
  5. no payment/stamp is needed

*due to agreements between Norway and EU, citizens of Norway go through the same procedure as students from EU countries

Students from countries that are not members of the European Union

WHAT DO YOU NEED TO APPLY

  1. application form – (must be completed in Polish)
  2. 4 colour, up-to-date photographs of the student-taken within past six months, size 35x45 mm
  • showing the eyes and face from the top of the head to the shoulders, so that the student’s image fills 70-80% of the entire frame
  • taken on a plain light background, in frontal position with open and looking straight ahead eyes. The photographs need to provide the student without head cover, with a relaxed facial expression and closed mouth. It also needs to present student’s natural skin colour
  • eyes of a student (especially pupils) need to be clearly shown, the line of the student’s eyes are parallel to the upper edge of the photograph


ATTENTION! Students who wear head cover according to their religious principles may have the photographs taken with the head cover, but the face must be visible
(in such a situation students must provide statement of belonging to a religious community)

  1. copy of passport (the original is to be shown at the Office)
  2. confirmation of being a Collegium Medicum student (it may take up to 5 days for the documents to be signed)
  3. proof of valid insurance coverage
  4. bank statements from the student’s bank account (in Poland) to confirm that the student has the financial means to cover his/her stay in Poland
  5. confirmation of covering the costs of issuing the temporary stay card (340 PLN) and the costs of the card itself (50 PLN)
